What pressure does to the heart and arteries

Blood pressure is the force blood exerts on artery walls. When it is persistently high, two things happen. The arteries respond to sustained stress by thickening and stiffening, which accelerates atherosclerosis and further raises pressure. And the left ventricle, pumping against greater resistance, thickens — left ventricular hypertrophy — which eventually stiffens the chamber and impairs filling.

That sequence explains why hypertension contributes to so many different problems: heart attack, stroke, heart failure, atrial fibrillation, kidney disease, and vascular cognitive impairment.

  • Damage accumulates silently over years before any symptom appears.
  • Risk rises continuously with pressure — there is no sharp threshold where harm begins.
  • The combination of high pressure with diabetes, smoking, or high cholesterol multiplies risk rather than adding to it.

Targets and how they are set

The 2017 ACC/AHA guideline defines stage 1 hypertension at 130 to 139 systolic or 80 to 89 diastolic, and stage 2 at 140/90 or above. For many adults with hypertension, a treatment target below 130/80 mmHg is recommended, informed by trials showing fewer cardiovascular events with more intensive control.

Targets are individualised. Older adults with frailty, people prone to falls or dizziness, and those with certain kidney conditions may have different goals, because aggressive lowering carries its own risks. Guidelines from other countries set slightly different thresholds, which is a reflection of how the same evidence can be weighted differently rather than a contradiction.

Lifestyle changes and their measured effect

Non-drug approaches have quantified effects, and they combine.

  • DASH eating pattern — around 8 to 11 mmHg systolic reduction in trials.
  • Sodium reduction — typically 5 to 6 mmHg, and more in salt-sensitive people; target under 2,300 mg a day with 1,500 mg ideal for many with hypertension.
  • Aerobic activity — roughly 5 to 8 mmHg with regular training.
  • Weight loss — approximately 1 mmHg per kilogram lost.
  • Alcohol reduction — several mmHg in people who drink regularly.
  • Increased dietary potassium — from fruit, vegetables, and legumes, unless kidney disease or certain medications make that inappropriate.

These are not small numbers. Combined, they can equal or exceed the effect of a single medication, though many people need both.

Medication, adherence, and monitoring

When pressure remains above target, medication is added. The main classes are ACE inhibitors, angiotensin receptor blockers, calcium channel blockers, and thiazide-type diuretics, with others used in specific circumstances. Many people need two or more agents. Selection depends on age, ethnicity, kidney function, other conditions, and pregnancy status, and dosing is set by the prescriber.

Adherence is the weak point. Because hypertension has no symptoms, people commonly stop when they feel well or when a side effect appears. Alternatives usually exist, and combination pills can reduce the number of tablets.

  • Take medication at a consistent time and link it to an existing routine.
  • Report side effects rather than stopping quietly.
  • Bring home readings to appointments — they guide adjustment better than clinic values alone.
  • Ask about interactions, including with common over-the-counter pain relievers and decongestants.

Warning signs and when to seek urgent care

High blood pressure itself is usually silent, but the events it contributes to are not. Knowing what to act on matters.

  • Call emergency services for chest pain or pressure, pain spreading to arm, jaw, or back, sudden severe breathlessness, or fainting.
  • Stroke warning signs — sudden face drooping, arm weakness, or speech difficulty. Note the time and call immediately.
  • Hypertensive crisis — a reading at or above 180/120 mmHg. Recheck after five minutes; if it remains that high with symptoms such as chest pain, breathlessness, weakness, or vision change, seek emergency care. Without symptoms, contact your clinician promptly.
  • Prompt appointment — persistently rising readings, new swelling in the legs, or worsening breathlessness on exertion.
