The two families of interaction
Changes in drug levels (pharmacokinetic). One substance changes how much of another is absorbed, how fast the body breaks it down, or how it is cleared. Most of these involve liver enzymes — the CYP450 family — or transport proteins such as P-glycoprotein.
- Inhibitors block the clearing enzyme, so drug levels rise. Grapefruit juice, clarithromycin, and some antifungals do this.
- Inducers ramp the enzyme up, so drug levels fall and the medicine underperforms. Rifampin, carbamazepine, and St John’s wort are classic inducers.
