What is Low-Dose Aspirin?
Low-dose aspirin permanently blocks an enzyme inside platelets, so they cannot produce the signal that makes them clump. Blood clots form less readily, which lowers the chance of a clot blocking a coronary or brain artery. It is well established for people who have already had a heart attack or stroke; for prevention in people who have not, guidance has narrowed considerably.

How it generally works
Platelets are the cell fragments that plug damaged blood vessels. To recruit each other they make a messenger called thromboxane A2, using an enzyme called COX-1. Aspirin blocks that enzyme irreversibly — and because platelets have no nucleus, they cannot make new enzyme. A single exposure disables a platelet for its whole lifespan, which is roughly a week.
That is why a small amount, taken daily, produces a steady antiplatelet effect: each day a fraction of new platelets is disabled as it enters circulation. Higher amounts of aspirin add pain and fever relief but no extra antiplatelet benefit, and they do add stomach and bleeding risk — which is the reason the cardiovascular use is at a low amount.

Most people tolerate Low-Dose Aspirin well, and many effects settle as the body adjusts. Tell your prescriber or pharmacist about anything that persists or bothers you.

Serious side effects

- Black, tarry, or bloody stools, or vomiting material that looks like coffee grounds — gastrointestinal bleeding
- Sudden severe headache, weakness on one side, or confusion — possible bleeding into the brain; call 911
- Wheezing, facial swelling, or hives after taking aspirin — aspirin-exacerbated respiratory disease or hypersensitivity, more common in people with asthma and nasal polyps
- Ringing in the ears with confusion — a sign of salicylate excess, usually from higher amounts
- Unexplained fatigue and pallor — may reflect slow blood loss and anemia
Important warnings
- Bleeding — aspirin increases gastrointestinal bleeding and, less commonly, bleeding in the brain. Risk rises with age, prior ulcer, alcohol, and combination with other blood thinners or NSAIDs.
- Primary prevention has changed — the USPSTF now advises against starting aspirin for primary prevention at age 60 and above, and describes only a small net benefit for some adults aged 40 to 59 with elevated cardiovascular risk. This is an individual conversation, not a default.
- Reye syndrome — aspirin is not given to children or teenagers with a viral illness such as flu or chickenpox because of this rare but serious condition.
- Do not stop before surgery or dental work on your own — for some people, especially those with a recent stent, stopping carries more risk than bleeding does. Ask the team who placed the stent.
- Aspirin allergy or aspirin-exacerbated respiratory disease — a history of wheezing, nasal polyps, and aspirin sensitivity is a contraindication.
- Pregnancy — low-dose aspirin is sometimes prescribed deliberately in pregnancy for preeclampsia risk, but that is a specific medical decision and not the same as taking it on your own.
Drug interactions

- Anticoagulants such as warfarin, apixaban, and rivaroxaban — substantially increased bleeding risk; the combination is used only when there is a clear reason.
- Other NSAIDs, especially ibuprofen — can block aspirin’s access to the platelet enzyme and reduce its cardiovascular benefit, and add gastrointestinal risk.
- Corticosteroids — additive risk of stomach ulceration.
- SSRIs and SNRIs — increase bleeding risk when combined with aspirin.
- Alcohol — adds to gastrointestinal bleeding risk.
- Some herbal products including ginkgo, garlic supplements, and high-dose fish oil — may add to bleeding risk; include them on your medication list.

Storage
- Store in a dry place at room temperature; aspirin breaks down in humidity and develops a vinegar smell when it does.
- Discard tablets that smell strongly of vinegar or look crumbly, even before the expiry date.
- Keep enteric-coated tablets in the original container and out of reach of children — aspirin poisoning in children is serious.

Should healthy adults take a daily aspirin?
Generally no, and this has changed. In 2022 the USPSTF recommended against starting aspirin for primary prevention at age 60 and older, and found only a small net benefit for some adults aged 40 to 59 whose cardiovascular risk is elevated. Bleeding risk rises with age while the preventive benefit does not. If you already take it, ask before stopping.
Is enteric-coated aspirin gentler on the stomach?
The coating reduces direct contact with the stomach lining, but most aspirin-related ulcers come from its systemic effect on protective prostaglandins, which the coating does not change. Studies have not shown a clear reduction in serious bleeding with enteric coating, and absorption can be less predictable. Discuss stomach protection with your prescriber instead of relying on the coating.
Can I take ibuprofen if I am on daily aspirin?
With care. Ibuprofen can occupy the same site on the platelet enzyme and physically block aspirin from reaching it, reducing aspirin’s heart protection, and both together increase stomach bleeding. FDA has issued guidance on separating the timing. Ask your pharmacist, and consider whether a different pain reliever fits your situation better.
How long does aspirin keep working after I stop?
Because aspirin disables platelets permanently, the effect fades only as your body makes new ones — roughly seven to ten days for full turnover. This is why surgeons ask about aspirin well in advance of procedures, and why a single missed day rarely matters much. Never plan a stop on your own if you have a stent.
